My first instinct was to use a circular polarizer, but you seem to have done that. Are we sure that all the filters and front and rear lens elements are clean?

Also, you should take the UV filter off when using the polarizer. Stacking filters can result in bad reflections being captured in the image, as well as vignetting at wide angles.
